COMMUNITY HEALTH REPRESENTATIVE (CHR)

Scope:

Under the supervision of the Health Director/Manager, the CHR will attend to physical health related issues and facilitate access to external health services for Big Grassy members.

Responsibilities and Tasks: ( may not be limited to:)

1/ Ensuring the efficiency and cost effectiveness of the medical transportation program

2/ Maintain statistical data of program activities (# of clients seen, referrals made, etc.)

Submit monthly program activity narrative and statistical reports

3/ Consulting w/Specialists, Physicians on medication procedures, submitting & obtaining referrals. Attaining treatment centre space for clientele, etc. Answering calls from clients for various concerns with regard to health  OHIP, FNIB, Disabilities .

4/ Coordination and accommodating visiting nurses, specialist health technicians through Fort Frances Tribal Area Health Services. (FFTAHS)

5/ Ensuring Health Canada invoicing is compliant with their re-imbursement criteria and regulations.

Key Working Relationships:

Esiniiwab Health Center Director/Manager – Reporting to and maintaining daily communication as required.

Health Team –Daily/Weekly (NNADAP, Early Childhood Worker, Community Wellness Worker.)

Community Members: Providing essential services and advocacy

FFTAHS – Facilitating and accommodating visitations to the Health Center and client services.

External Health Services: Maintaining communication with Doctors, Hospital staff and other providers as necessary.

Medical Drivers:  Ensuring availability and provision of transportation for Big Grassy members to appointments.

 

PRIMARY QUALIFICATIONS:

*demonstrated computer competence and working knowledge of MS Word, Excel, Outlook.

*a demonstrated ability to work well as a member of a team, understanding and adhering to roles and responsibilities within the First Nation organization

*Certification or previous training and demonstrated abilities in this position

*ability to speak Ojibway is an asset

*must provide current “Vulnerable Sector” check;

*must have a valid driver’s license and be willing to travel as needed

 

This position is full time with a 3-month probationary period.

 

SALARY:   Will commensurate with experience.
